1. Recent passport size photographs of main member, spouse and children under the age of 26 years.
2. Copy of CNIC, and Family Registration Certificate (FRC).
3. Evidence of last month’s payment
4. For the issuance of main member’s parents card kindly provide their CNIC and Passport size photographs.
